What is schema Therapy?

Schema therapy developed from cognitive behavioural therapy, but focuses on long-standing life patterns rooted in core emotional needs that were not met in childhood and adolescence. Deep beliefs called "early maladaptive schemas", such as abandonment, defectiveness, emotional deprivation or self-sacrifice, and the "modes" that appear when they are triggered (for example the hurt child, the punitive inner voice or the detached protector) are at the centre of the work. The therapist uses techniques such as imagery rescripting of past experiences, chair work, testing schemas against reality and trying new behaviours; the therapy relationship also serves as a "reparenting" function within clear limits. The strongest scientific evidence is in borderline personality disorder; there are promising but more limited studies in other personality disorders, chronic depression and recurring relationship problems. It takes longer than classic CBT, and it matters that the therapist has specific training in it.

How is schema Therapy done?

  1. First meetings assessing history, life events and schema questionnaires
  2. Formulating the schemas and modes together with the person
  3. Working at an emotional level with experiential techniques such as imagery and chair work
  4. Testing the thoughts that feed schemas and trying new behaviours in daily life
  5. Strengthening the healthy adult mode and gradually spacing out sessions
