What is scoliosis Surgery?

Scoliosis is a curve of the spine into an S or C shape when seen from behind (not from the side), with the vertebrae also rotating on their own axis. The most common type is idiopathic scoliosis, whose cause is unknown and which appears in adolescence. The size of the curve is measured on a standing X-ray as the Cobb angle. Mild curves are only monitored; in children who are still growing, moderate curves are treated with a brace. Surgery is usually considered for curves above about 45–50 degrees in growing teenagers, or for large curves that keep progressing even after growth has finished. The most common operation is posterior spinal fusion: screws are placed in the vertebrae from the back, the curve is corrected with rods and that section is fused. In young children, growing rods that allow growth are used, and in selected teenagers vertebral body tethering, which aims to guide the curve without fusion, is used; the long-term results of this last method are still being studied. In adult scoliosis, pain, nerve compression and balance problems matter more than the angle when deciding on surgery.

How is scoliosis Surgery done?

  1. Full-length standing spine X-rays, bending films and MRI of the spinal cord if needed
  2. Lung function tests, blood preparation and anaesthesia assessment
  3. Neuromonitoring throughout surgery under general anaesthesia to watch spinal cord function
  4. Placing screws in the vertebrae, correcting the curve with rods and adding bone grafts for fusion
  5. Intensive care or close observation, early mobilisation and pain management
