What is sex Therapy?

Sex therapy is a usually short-term, goal-focused therapy that works on the psychological, relational and learned sides of problems that make sex difficult. The first step is to investigate, with a urologist or gynaecologist, whether there is a medical cause such as a hormonal, vascular or neurological problem or a medication side effect. A detailed sexual history is then taken, and myths and fears are discussed (psychoeducation). Treatment uses cognitive behavioural techniques, gradual "sensate focus" exercises the couple do together, step-by-step exposure work that reduces fear in vaginismus, and behavioural techniques to notice and manage arousal in premature ejaculation. There is no physical contact or examination in sessions; exercises are done at home, in the privacy of the person or couple. Structured therapy programmes for vaginismus are reported to enable many women to have intercourse; for premature ejaculation and erectile dysfunction, behavioural methods often work better combined with medication. Response varies from person to person.

How is sex Therapy done?

  1. Urology or gynaecology assessment to rule out medical causes
  2. A detailed sexual history individually and, if needed, with the couple
  3. Psychoeducation on the sexual response cycle and how the problem is maintained
  4. Planning gradual exercises to do at home and reviewing them in sessions
  5. Working on anxiety, shame and relationship problems; a relapse prevention plan
