What is anal Fistula Surgery?

An anal fistula is a small tunnel between a gland opening inside the anal canal and the skin around the anus, usually after an abscess in that area. It is noticed as an opening in the skin that closes and reopens and drains blood or pus. A fistula does not close by itself with medicines; its treatment is surgical. The choice of operation depends on how the tunnel relates to the sphincter muscles that keep the anus closed. For simple fistulas that cross only a small part of the muscle, fistulotomy, which opens and flattens the tunnel, has a high healing rate. For complex fistulas that cross a large part of the muscle, sphincter-sparing methods are used to lower the risk of leakage: a thin rubber or thread loop (seton) passed through the tunnel to drain it or slowly cut through it; the LIFT procedure, which ties off and closes the tunnel between the muscles; an advancement flap covering the internal opening with healthy tissue; or laser treatment inside the tunnel. Sphincter-sparing methods have higher recurrence rates than fistulotomy, and more than one operation may be needed.

How is anal Fistula Surgery done?

  1. Examination of the anus and the position and number of external openings
  2. For complex or recurrent fistulas, mapping the tunnel with pelvic MRI or endoanal ultrasound
  3. Gently following the tunnel with a probe under spinal or general anaesthesia or sedation, and finding the internal opening
  4. Fistulotomy, seton, LIFT, advancement flap or laser depending on its relation to the muscle
  5. Wound-care teaching and, for patients with a seton, planning the next stage
