What is anal Fissure Treatment?
An anal fissure is a lengthwise tear in the thin skin lining the anal canal. It starts after a hard stool or severe diarrhoea; the pain of the tear makes the internal sphincter muscle spasm, the spasm reduces blood flow to the area, and the wound does not heal. Treatment aims to break this vicious circle. Most acute fissures that have lasted less than six weeks heal with a high-fibre diet, plenty of fluids, stool softeners and warm sitz baths. If they do not heal, prescription creams that relax the sphincter muscle are used. For fissures that do not respond to creams or become chronic, a botulinum toxin (Botox) injection into the internal sphincter relaxes the muscle temporarily for a few months and gives the wound time to close. When a lasting solution is needed, lateral internal sphincterotomy surgery cuts a small part of the internal sphincter in a controlled way; this has the highest healing rate but carries a small risk of leaking gas or stool. Fissures that are not in the midline of the anus or that do not heal should be investigated for other diseases.
How is anal Fissure Treatment done?
- Listening to the symptoms and a gentle external examination; postponing internal examination if pain is severe
- First-line treatment with stool softeners, a high-fibre diet and sitz baths
- If not enough, a prescription muscle-relaxing cream for a set period
- For a fissure that does not heal, Botox injection into the internal sphincter under sedation or brief anaesthesia
- When a lasting solution is needed, sphincterotomy cutting a small part of the internal sphincter; removal of an associated skin tag or polyp if needed
