What is root Canal Retreatment?
Root canal retreatment is done when, months or years after the first root canal, the tooth develops pain, swelling or a fistula (a pimple on the gum that drains), or when an X-ray shows that inflammation at the root tip has not healed. Common reasons for failure are a missed extra canal, canals that were not cleaned or filled well enough, a leaking filling or crown on top, and new decay. During retreatment the filling or crown on the tooth is removed if needed, the old canal filling material and any post inside the canal are taken out, and the canals are reshaped, disinfected and filled again. Dental CT and magnification (a microscope) help to find missed canals and to assess obstacles such as a broken instrument. The chance of success may be lower than for the first treatment; if the root is fractured, the tooth usually cannot be saved.
How is root Canal Retreatment done?
- Examination, periapical X-ray and dental CT if needed to find the cause of failure
- Discussing whether the tooth can be saved and the alternatives
- Local anaesthesia, isolating the tooth with a rubber dam and removing the restoration on top
- Removing the old canal filling and any post or obstacles; cleaning and disinfecting the canals again
- Filling the canals and sealing the tooth with a leak-proof filling or crown
