What is vaginismus Treatment?

Vaginismus is the involuntary tightening of the muscles around the vagina, with intense anxiety or fear, when anything (a partner, a tampon or an examination instrument) is expected to enter the vagina. In current classifications it is covered under genito-pelvic pain/penetration disorder. It has nothing to do with a person's wishes or willpower, it is nothing to be ashamed of, and it is one of the problems that respond well to treatment. The first step is to rule out physical causes that can cause pain, such as infection, hymen structure, skin diseases or hormonal dryness, with a gentle examination done with the person's consent. Then sex therapy (cognitive behavioural approaches and work to reduce anxiety), accurate information about the body and sexuality, gradual exercises progressing under the person's own control and, if needed, pelvic floor physiotherapy are planned together. If there is a partner, their involvement is often helpful. If there is an accompanying anxiety disorder or a history of trauma, psychiatric support is added. In resistant cases, botulinum toxin injection into the muscle has been tried, but evidence for it is limited and it is considered together with therapy, not on its own.

How is vaginismus Treatment done?

  1. A confidential first meeting: the history of the complaint, expectations and the relationship with any partner
  2. A gynaecological assessment with the person's consent and at their own pace, starting with only an external examination if needed
  3. Information about the body, sexual response and the anxiety cycle
  4. Relaxation, breathing and gradual touch exercises; pelvic floor physiotherapy if needed
  5. Stages carried out together with the partner and joint review of progress
