What is tear Duct Surgery (DCR) and Probing?

Tears drain through small openings at the inner corner of the eye into the tear sac, and from there through a duct into the nose. When this duct is blocked, the eye waters and crusts constantly, and the sac can become inflamed (dacryocystitis), causing a painful swelling at the inner corner of the eye. In newborns, blockage due to a membrane at the nasal end of the duct that has not opened is common, and in most babies it opens by itself with massage within the first year. If it does not, the duct is usually opened with a fine probe under a short anaesthetic (probing); a temporary silicone tube may be placed in the duct if needed. In adults, the blockage is usually related to age, past infections or injuries to the nose and face; here the main treatment is dacryocystorhinostomy (DCR). In DCR a new passage is made between the tear sac and the nasal cavity. It can be done through a small skin cut between the eye and nose (external approach) or with an endoscope through the nose without any cut; the endoscopic method is done by eye doctors and ENT specialists.

How is tear Duct Surgery (DCR) and Probing done?

  1. Examination and testing whether the tear ducts are open by flushing them with saline
  2. Contrast imaging and endoscopic examination of the nose if needed
  3. In babies, probing under a short general anaesthetic, with a silicone tube if needed
  4. In adults, external or endoscopic DCR under general or local anaesthesia
  5. If a silicone tube was placed, removing it in the clinic weeks or months later
