What is prostate Biopsy (MRI Fusion)?

A prostate biopsy is the way to find out for certain whether there is cancer when the PSA is raised, a firm area is felt on examination or a suspicious spot is seen on a prostate MRI. Many small tissue samples are taken from different areas of the prostate with a fine needle advanced through the rectum or through the skin between the anus and the scrotum (transperineal). In MRI fusion biopsy, previously taken multiparametric prostate MRI images are overlaid on the ultrasound image during the procedure with the help of software, so targeted samples can be taken from the suspicious area seen on MRI. This approach aims to increase the chance of finding significant cancers while reducing unnecessary diagnosis of insignificant, slow-growing tumours; usually systematic samples are also taken from the rest of the prostate in addition to targeted ones. The transperineal route has been found to have a lower infection risk than biopsy through the rectum. A biopsy is not a treatment by itself; its result is assessed together with the Gleason score and risk group to plan monitoring or treatment.

How is prostate Biopsy (MRI Fusion) done?

  1. Deciding whether a biopsy is needed with PSA, a rectal examination and multiparametric prostate MRI
  2. Adjusting blood thinners together with the doctor, a urine test and culture if needed
  3. Preventive antibiotics; local anaesthesia, sedation or general anaesthesia
  4. Taking targeted samples from the suspicious spot and systematic samples from other areas of the prostate under ultrasound or MRI-ultrasound fusion guidance
  5. Short observation, discharge after passing urine and a follow-up appointment for the pathology result
