What is prolotherapy (Dextrose Injections)?

Prolotherapy is repeated injection of a solution, usually containing dextrose (sugar), into ligament and tendon attachments thought to cause chronic pain, or into a joint. The idea behind it is that the solution triggers a controlled, mild inflammatory response that stimulates tissue repair; however, this mechanism has not been fully proven, and there may be other reasons it reduces pain. Small and medium-sized studies in knee osteoarthritis, tennis elbow and some chronic low back pain report reduced pain in some patients, but the number and quality of studies and the consistency of results are limited, and its advantage over placebo is not clear in every case. Prolotherapy is therefore not a replacement for standard treatments but an option that can be considered, after discussing risks and benefits, in selected patients who have not responded to exercise and physiotherapy. It should be done by a doctor under sterile conditions and preferably with ultrasound guidance.

How is prolotherapy (Dextrose Injections) done?

  1. Physical medicine examination; clarifying the source of pain with imaging and examination
  2. Reviewing treatments already tried and discussing the level of evidence with the patient
  3. Cleaning the skin with antiseptic and local anaesthetic if needed
  4. Injection with a fine needle under ultrasound guidance or at anatomical points
  5. Brief observation; planning repeat sessions a few weeks apart and an exercise programme
