What is micro-TESE (Testicular Sperm Extraction)?

Azoospermia means no sperm is seen in the semen in at least two separate semen analyses. It can be caused by blocked sperm ducts (obstructive) or by very low sperm production in the testis (non-obstructive). In non-obstructive azoospermia, even if most of the testis does not make sperm, production may continue in some small areas. In micro-TESE, the testis is opened under general or spinal anaesthesia and examined under magnification with an operating microscope; fuller-looking tubules, which are more likely to contain sperm, are chosen and small pieces are removed. At the same time an embryologist searches these pieces in the lab until sperm is found. Sperm that is found is used straight away for ICSI in IVF, or frozen. The aim is a higher chance of finding sperm while removing less tissue than with classic random biopsies. Sperm may not be found in every man, however; the chance varies a great deal with the cause of azoospermia, hormone levels and genetic findings, and this should be discussed openly before surgery.

How is micro-TESE (Testicular Sperm Extraction) done?

  1. Confirming azoospermia with at least two semen analyses
  2. Hormone tests such as FSH, LH and testosterone, testicular ultrasound and genetic tests (karyotype, Y chromosome microdeletion)
  3. Considering hormone treatment or varicocele treatment if needed
  4. Timing with the IVF centre; examining the testis under the microscope under anaesthesia and taking tubule samples
  5. Searching for sperm in the lab, then using or freezing any sperm found
