What is undescended Testicle Surgery (Orchiopexy)?
An undescended testicle means one or both testicles are not in the scrotum at birth in a baby boy; it is more common in babies born early. Some testicles come down by themselves in the first few months; after about six months, the chance of this becomes very small. In a testicle that stays outside the scrotum, closer to body temperature, the cells that will make sperm can be damaged over time; the risk of testicular cancer later in life also rises slightly, and testicular torsion or an inguinal hernia are more common. For this reason most international guidelines recommend that, if the testicle has not come down, surgery is done after six months and preferably before 12, and at the latest before 18, months of age. For testicles that can be felt, the testicle is freed through a small cut in the groin (sometimes the scrotum), any accompanying hernia sac is closed, and the testicle is placed in a pocket made in the scrotum. For testicles that cannot be felt, keyhole surgery is first used to see whether the testicle is inside the abdomen; if needed, the procedure is done in two stages. The operation is done by paediatric surgeons and urologists who work in paediatric urology; the paediatrician who follows the baby plays a key role in diagnosis and referral.
How is undescended Testicle Surgery (Orchiopexy) done?
- Noticing at a paediatric check-up that the testicle is not in the scrotum, and monitoring in the first months
- Paediatric surgery or paediatric urology assessment if it has still not come down after six months
- Determining whether the testicle can be felt and telling it apart from a retractile testicle
- Bringing the testicle down through a groin or scrotal cut under general anaesthesia; keyhole surgery for a testicle that cannot be felt
- Discharge and, over the following months, checks that the testicle stays in place and grows
