What is BPPV Treatment (Epley Manoeuvre)?

Benign paroxysmal positional vertigo (BPPV), often described as "loose crystals", is the most common cause of dizziness. Tiny calcium crystals (otoconia) in part of the balance organ of the inner ear come loose and slip into one of the semicircular canals. When the person lies down, turns over in bed or tips their head up or down, they get a brief (usually under a minute) but intense spinning feeling. The diagnosis is made when the doctor sees a characteristic involuntary eye movement (nystagmus) while placing the head in certain positions. Treatment uses repositioning manoeuvres, in which the head is turned at set angles in a set order, rather than medicine: the best known are the Epley and Semont manoeuvres for the posterior canal, and the Gufoni and barbecue (Lempert) manoeuvres for the horizontal canal. The effectiveness of the Epley manoeuvre for posterior canal BPPV has been shown in controlled studies; most people improve clearly after one or a few treatments. But not every dizziness is BPPV; the right diagnosis is needed before a manoeuvre is done.

How is BPPV Treatment (Epley Manoeuvre) done?

1. Taking the history: when the dizziness happens, with which movement and for how long
2. Neurological examination and ruling out dangerous causes such as stroke
3. Dix-Hallpike and supine roll tests to find which canal and which ear is affected
4. Doing the manoeuvre suited to the affected canal (Epley, Semont, Gufoni or barbecue), waiting a set time in each position
5. A short rest, a repeat in the same session if needed, and a check test
