What is fracture Surgery (Plates, Nails and Screws)?

Fracture surgery brings the broken pieces of bone into the right position (reduction) and holds them with metal implants until they heal. What people often call a "platinum" plate is usually a plate made of titanium or stainless steel with screws. In long bones such as the thigh and shin bones, a nail placed in the marrow of the bone (intramedullary nail) can be used; for small fragments, screws or wires alone; and for severe soft tissue injuries, an external fixator attached from outside the skin. A cast or splint wraps the bone from outside and keeps it still, and is enough for most fractures if the pieces stay in place. Surgery is preferred when the pieces have shifted, the fracture extends into a joint, the bone has come through the skin, or staying in a cast for a long time would cause major problems; the aim is for the bone to heal at the right angle and for the joint to be moved early. The body, not the operation, heals the bone; the metal only holds the pieces in place during this time.

How is fracture Surgery (Plates, Nails and Screws) done?

  1. Examination with checks of blood vessels and nerves; X-ray and CT if needed to determine the type of fracture
  2. Deciding on timing (urgent for open fractures; for closed fractures with a lot of swelling, waiting for it to go down)
  3. General, spinal or regional nerve block anaesthesia
  4. Setting the fracture closed or through an incision, with X-ray (fluoroscopy) checks during surgery
  5. Fixing with a plate and screws, a nail, wires or an external fixator; closing the incision and a splint if needed
