What is vocal Cord Surgery (Microlaryngeal Surgery)?

The vocal cords are two thin muscle-and-membrane structures in the larynx that vibrate to make the voice. Overusing or misusing the voice, smoking and reflux can cause benign swellings on the vocal cords. Nodules are usually on both sides, symmetrical and callus-like; they are common in teachers, singers and children, and most shrink with voice therapy. Polyps are more often on one side, can develop after a sudden strain or bleed, and usually need surgery. Vocal cord cysts and Reinke's oedema, seen in smokers, are also treated surgically. In microlaryngeal surgery the patient is asleep, a metal tube (laryngoscope) placed through the mouth gives access to the vocal cords, and only the lesion is removed under a microscope with very fine instruments or a laser; the vibrating surface of the vocal cord is preserved as much as possible. The removed tissue is always sent for pathology. Surgery alone is not enough: if habits that strain the voice continue, the problem can come back, so voice therapy before and after surgery is part of treatment.

How is vocal Cord Surgery (Microlaryngeal Surgery) done?

  1. Endoscopic examination of the larynx and assessment of vocal cord vibration with stroboscopy
  2. For nodules, voice therapy first; managing factors such as reflux, allergy and smoking
  3. Placing a laryngoscope through the mouth under general anaesthesia
  4. Removing the lesion under the microscope with cold instruments or a laser and sending it to pathology
  5. Voice rest, then referral for voice therapy
