What is corneal Transplant (Keratoplasty)?

The cornea is the clear, dome-shaped window at the front of the eye. Advanced keratoconus, swelling from failure of the cornea's inner layer (for example Fuchs dystrophy or swelling after eye surgery), corneal injuries and white scars left after infection can cloud the cornea and seriously reduce vision. In a corneal transplant, the diseased tissue is replaced with a healthy cornea donated after death and checked by an eye bank. In a full-thickness transplant (penetrating keratoplasty) all layers of the cornea are replaced. In layered transplants only the diseased part is replaced: an anterior layered transplant (DALK) is preferred in keratoconus, and posterior layered transplants (DMEK, DSAEK) when the inner layer fails. Layered methods usually have advantages in rejection risk and healing time. Vision can take months to settle after a transplant, and most patients still need glasses or contact lenses.

How is corneal Transplant (Keratoplasty) done?

  1. Assessment with corneal topography, thickness measurement and an inner-layer cell count
  2. Choosing the type of transplant and waiting for a donor cornea through an eye bank
  3. Removing the diseased tissue under local or general anaesthesia
  4. Placing the donor cornea with stitches or, in posterior layered transplants, with the support of an air bubble
  5. Frequent check-ups, anti-rejection drops and gradual removal of stitches over time
