What is Achilles Tendon Rupture Treatment and Surgery?

The Achilles tendon is the body's strongest tendon; it connects the calf muscles to the heel bone and lets you rise onto your toes, jump and climb stairs. It usually ruptures between the ages of thirty and fifty in people who play sport occasionally, during a sudden jump or sprint; people describe a feeling as if someone kicked their heel or a stone hit it, and sometimes a snapping sound. The diagnosis is usually made by examination; an important sign is that the foot does not move down when the calf is squeezed, and ultrasound or MRI shows where the rupture is and the gap between the ends. There are two treatment routes. In surgery, the tendon ends are joined with strong stitches through an open or small incision (percutaneous, mini-open). In non-surgical treatment, a boot or cast with a heel raise keeps the toes pointing down and lets the tendon ends heal by themselves. With current rehabilitation programmes using early weight-bearing and movement, the results of the two methods are similar; surgery has a lower risk of re-rupture but a higher risk of wound problems.

How is Achilles Tendon Rupture Treatment and Surgery done?

  1. Examination with the calf squeeze test and feeling for a gap along the tendon
  2. Ultrasound or MRI if needed to find the site of the rupture and measure the gap between the ends
  3. Deciding between surgery and non-surgical treatment
  4. In surgery, spinal or general anaesthesia; stitching the tendon ends through an open or small incision
  5. A splint or heel-raise boot with the toes pointing down, and a rehabilitation plan
