What is cervical Disc Herniation Surgery?
A herniated disc in the neck is when the inner part of a disc between the neck vertebrae bulges out and presses on nerve roots running to the arm or on the spinal cord. Most patients improve with medication, physiotherapy and time, without surgery. Surgery is considered when arm pain does not settle despite treatment for long enough, when muscle weakness is getting worse, or when there are signs of pressure on the spinal cord (myelopathy). The most common method is anterior cervical discectomy and fusion (ACDF): the disc is reached through a small cut at the front of the neck, the herniation and the whole disc are removed, and a cage is placed in the space so the two vertebrae fuse. In selected patients, the disc can be removed by the same route and replaced with an artificial disc that aims to keep movement (cervical disc replacement). For some herniations that bulge to the side, the nerve root can be freed from the back of the neck under a microscope (posterior foraminotomy). In Turkey, neck disc operations are done mainly by neurosurgeons and also by orthopaedic surgeons who work in spine surgery; neurologists are involved in diagnosis, nerve conduction tests and non-surgical treatment.
How is cervical Disc Herniation Surgery done?
- Neurological examination, MRI and EMG if needed to find the level of compression and its effect on the arm nerves
- Assessing the response to non-surgical treatment and deciding on surgery
- General anaesthesia; a small cut at the front or back of the neck
- Removing the herniated disc and bony spurs under the microscope to free the nerve
- Fusion with a cage and, if needed, a plate, or placement of an artificial disc, then usually 1–2 days of observation in hospital
