What is electroconvulsive Therapy (ECT)?
Electroconvulsive therapy (ECT), popularly known as "shock therapy", is given today in a very different way from old film scenes. The patient is first put to sleep with a brief general anaesthetic and given a muscle relaxant; then a very brief electrical stimulus through electrodes on the scalp induces a controlled seizure in the brain lasting from a few seconds to a minute. Because the muscles are relaxed, there is no marked shaking of the body; the seizure is monitored with EEG, and heart rhythm and oxygen levels with a monitor. The treatment is thought to work by changing how brain networks and chemical messengers function. ECT is used for severe depression, depression with psychotic features, situations needing a rapid response such as refusing to eat and drink or a high suicide risk, catatonia, severe mania and some psychotic disorders that do not respond to medicines. In severe depression, response rates have been found to be higher and faster than with medication; however, most patients need medication or maintenance ECT afterwards to keep the effect. In Turkey, ECT is done in hospitals where psychiatry and anaesthesia teams work together, with written informed consent from the patient or their legal representative.
