What is ovarian Cyst Surgery (Laparoscopic Cystectomy)?

Most ovarian cysts are functional cysts linked to the ovulation cycle and go away by themselves within a few menstrual cycles, so not every cyst needs surgery. Surgery is planned for cysts that do not shrink or that grow despite a few months of monitoring, or that cause marked symptoms; for dermoid cysts containing tissue such as fat or hair; for cysts that make the ovary twist around itself (torsion) or that burst and cause internal bleeding; and for masses whose ultrasound appearance or tumour markers raise suspicion. In people of reproductive age, the aim is to remove the cyst and keep the ovary (cystectomy); after menopause or with suspicious masses, the whole ovary may need to be removed. It is usually done laparoscopically, and the removed tissue is always sent for pathology. Torsion and internal bleeding are emergencies that need urgent surgery. Chocolate cysts caused by endometriosis need a different approach and are covered on a separate page.

How is ovarian Cyst Surgery (Laparoscopic Cystectomy) done?

  1. Transvaginal or abdominal ultrasound; if needed, a repeat ultrasound a few weeks later to see how the cyst changes
  2. Tumour markers and MRI where needed
  3. Deciding between ovary-sparing surgery and removal of the ovary according to the cyst type, age and pregnancy plans
  4. Laparoscopy under general anaesthesia: separating the cyst from the ovary and removing it in a bag
  5. Reviewing the pathology result at the follow-up
