What is clinical Pilates?

Clinical Pilates is classic Pilates adapted for rehabilitation on the basis of an assessment. Unlike Pilates at a gym, the programme is planned by a physiotherapist according to the person's diagnosis, pain and movement restriction, risky movements are removed and difficulty is increased gradually. Working the deep abdominal and back muscles together, controlling the pelvis and spine in a neutral position, coordinating breath with movement and flexibility work are the core components. Exercises are done on a mat, with small equipment such as balls and bands, or on spring-resistance machines such as the reformer; sessions can be one-to-one or in small groups. In chronic non-specific low back pain, clinical Pilates has been shown to improve pain and function, but there is no evidence that it is clearly better than other well-planned exercise programmes. Adapted programmes are also used for neck pain, posture problems, after surgery, in pregnancy and after childbirth, and for osteoporosis.

How is clinical Pilates done?

  1. Doctor's diagnosis and a physiotherapist's assessment of posture, muscle strength, flexibility and pain
  2. Identifying movements to avoid and personal goals
  3. Teaching breathing, a neutral spine and activation of the trunk muscles
  4. A programme of mat and equipment exercises that gradually gets harder
  5. Home exercises and reassessment at set intervals
