What is corneal Cross-Linking for Keratoconus?
Keratoconus is a disease in which the cornea gradually thins and bulges forward into a cone, causing irregular astigmatism and blurred, distorted vision. It usually starts in the teenage years or early adulthood and can progress over the years. Corneal cross-linking aims to stiffen the tissue by creating new bonds between the collagen fibres in the cornea, and so to stop the disease from progressing. Riboflavin drops are soaked into the cornea, then ultraviolet A light is applied for a set time. In the classic method, the outermost cell layer of the cornea is removed (epithelium-off); evidence for the more comfortable epithelium-on methods is more limited. The main goal of cross-linking is to stop progression: it does not fully correct the existing shape problem and does not remove the need for glasses or lenses. It is therefore most useful when done early, in patients with documented progression and a cornea that is thick enough.
How is corneal Cross-Linking for Keratoconus done?
- Documenting progression with corneal topography and thickness measurements
- Numbing drops and, if needed, gentle removal of the top cell layer of the cornea
- Riboflavin drops at set intervals
- Controlled application of ultraviolet A light
- A protective soft contact lens and healing check-ups
