What is reflux Surgery (Laparoscopic Fundoplication)?

Reflux surgery is done for patients whose valve between the oesophagus and stomach is not working properly and who still have problems despite medication. The most common method is laparoscopic fundoplication: through a few small cuts in the abdomen, any stomach that has slipped through the opening in the diaphragm into the chest (hiatal hernia) is brought back down and the opening is narrowed with stitches. The top part of the stomach (the fundus) is then wrapped fully (360 degrees, Nissen) or partly (Toupet, Dor) around the lower end of the oesophagus. Before surgery, tests must show that reflux is really present and how the oesophagus moves, because surgery does not help patients whose symptoms resemble reflux but have other causes. In overweight patients, fundoplication may be less successful, and obesity surgery such as gastric bypass is then considered a more suitable option.

How is reflux Surgery (Laparoscopic Fundoplication) done?

  1. Gastroenterology and surgical assessment, reviewing symptoms and response to medication
  2. Upper endoscopy, 24-hour pH or pH-impedance monitoring and oesophageal pressure measurement (manometry); a barium X-ray if needed
  3. Bringing the hernia back into the abdomen laparoscopically under general anaesthesia and stitching the diaphragm opening
  4. Wrapping the top of the stomach fully or partly around the oesophagus
  5. Starting liquids the same or the next day, then gradually moving on
