What is trigger Finger Treatment and Surgery?

Trigger finger happens when the tunnel in the palm through which the finger-bending tendons pass (the A1 pulley) thickens and narrows, and the tendon catches as it passes through. When the finger bends or straightens it suddenly snaps like a trigger, sometimes stays bent and needs the other hand to straighten it; a painful lump may be felt in the palm at the base of the finger. It is worse in the mornings. Diabetes, rheumatoid arthritis and jobs with a lot of gripping raise the risk. Treatment is stepwise: in mild cases rest and a splint; commonly, a steroid injection into the narrowed sheath. The injection eases symptoms in many patients, but in some they come back. For fingers that do not respond to injection or remain locked, the sheath is opened through a small cut in the palm under local anaesthesia (A1 pulley release). Some centres also open the sheath with a needle without a cut (percutaneous release). Trigger thumb present from birth in children is a different condition and is assessed separately by paediatric orthopaedics.

How is trigger Finger Treatment and Surgery done?

  1. Examination to find which finger catches and at what stage; ultrasound if needed
  2. First step: a splint, adjusting activities and a steroid injection into the tendon sheath
  3. Planning surgery if there is no response
  4. Opening the narrowed sheath through a small cut in the palm under local anaesthesia and checking the finger moves freely
  5. Stitches, a small dressing and going home the same day
