What is heart Valve Repair and Replacement?
The heart has four valves that keep blood flowing in one direction; the aortic and mitral valves cause problems most often. When a valve narrows (stenosis), the heart has to strain to pump blood; when a valve does not close fully (regurgitation), blood leaks back. It may cause no symptoms for years; as it progresses, shortness of breath, tiring easily, chest pain, palpitations and fainting can occur. The diagnosis is made with echocardiography. The treatment decision is made by a heart team of cardiologists and cardiac surgeons together, according to how damaged the valve is, symptoms, effects on the heart muscle, age and other illnesses. In surgery through an open or small incision, the valve is repaired if possible (especially the mitral valve) or otherwise replaced with a mechanical or biological valve. For older patients with aortic stenosis or those at high surgical risk, transcatheter aortic valve implantation (TAVI) is used, in which a new valve is placed inside the old one with a catheter advanced from the groin artery. For some mitral and tricuspid leaks, methods that clip the valve leaflets together with a catheter, and for rheumatic mitral stenosis, opening the valve with a balloon (valvuloplasty), may be options.

- Examining the coronary arteries and the area around the valve with coronary angiography or CT
- The heart team choosing between surgery, TAVI or catheter repair
- For surgery, general anaesthesia and a heart-lung machine; for TAVI, usually placing the valve by catheter from the groin under sedation
- Monitoring in intensive care and on the ward, rhythm monitoring, an echocardiography check and referral for cardiac rehabilitation
