What is kyphoplasty and Vertebroplasty?

In osteoporosis the bones become thin and weak, so vertebrae can collapse with slight strain, even with everyday movements such as coughing or bending forward. These compression fractures cause sudden back pain, loss of height and, over time, a hunched back. Most fractures heal within a few weeks to a few months with pain relievers, short rest, a brace if needed and osteoporosis treatment. In vertebroplasty, bone cement is injected through a thick needle advanced into the broken vertebra from the back under imaging guidance, and the cement hardens to stabilise the vertebra. In kyphoplasty, a small balloon is first inflated inside the vertebra to create a space, which is then filled with cement; the aim is both to stabilise the vertebra and to regain part of the lost height. The scientific evidence is debated: some controlled studies have not shown a clear advantage of vertebroplasty over a sham procedure (placebo), while kyphoplasty has been reported to give benefit in pain and mobility in the first months compared with non-surgical treatment. The procedure is therefore recommended for selected patients whose new, painful fracture is confirmed on MRI and whose pain continues despite treatment.

How is kyphoplasty and Vertebroplasty done?

  1. Examination and MRI to find whether the fracture is new or old; ruling out other causes such as a tumour
  2. Bone density measurement and planning osteoporosis treatment
  3. Lying face down, with sedation or general anaesthesia and local anaesthesia
  4. Advancing a needle into the vertebra from the back under fluoroscopy; creating a space with a balloon in kyphoplasty
  5. Injecting the bone cement in a controlled way, a short observation and getting up
