What is endometriosis Surgery (Endometrioma Removal)?

Endometriosis is a long-term disease in which cells similar to the tissue lining the inside of the womb settle outside it and cause inflammation, adhesions and pain with each menstrual cycle. Old blood collecting inside the ovary gives the cyst dark brown contents, which is why an endometrioma is known as a chocolate cyst. The first step is usually medication; surgery is considered for pain that medicines cannot control, cysts that grow or look suspicious, involvement of the bowel, bladder or ureter, and some cases of infertility. Surgery is usually laparoscopic: the cyst wall is peeled away from the ovary, deposits on the abdominal lining are cut out or burned, and adhesions are released. Because some ovarian tissue can be lost with the cyst, how much tissue to touch is planned carefully, especially in people who want a pregnancy in future. Surgery may not remove the disease completely; it can come back over the years, so the treatment plan after surgery is also important.

How is endometriosis Surgery (Endometrioma Removal) done?

  1. Detailed examination, transvaginal ultrasound and MRI if needed to map the extent of the disease
  2. A blood test for ovarian reserve (AMH) and a discussion of pregnancy plans
  3. Joint planning with general surgery or urology if there is deep involvement
  4. Laparoscopy under general anaesthesia: peeling out the cyst, removing deposits and releasing adhesions
  5. Pathology examination of the removed tissue and a post-operative treatment plan
