What is deep Brain Stimulation (DBS)?
Deep brain stimulation (DBS), sometimes called a "brain pacemaker", is a treatment that delivers continuous, adjustable electrical stimulation through electrodes placed in specific nuclei deep in the brain. The electrodes are connected by a cable running under the skin to a battery-like device placed under the collarbone. Neurosurgeons do the operation; choosing patients, programming the device and long-term follow-up are the responsibility of neurologists. It is used most often in Parkinson's disease: in patients who respond well to medicines but whose medicine effect starts to fluctuate during the day, or who develop involuntary movements or tremor that medicines do not control, it can clearly reduce movement symptoms and lower the need for medication. It is also used in medicine-resistant essential tremor and dystonia, and to a limited extent in some patients with medicine-resistant epilepsy and obsessive-compulsive disorder. DBS does not stop the disease from progressing; its effect on symptoms that do not respond to medicines, such as balance problems, speech difficulty and memory problems, is usually limited. Patient selection is the most important factor in the result.
How is deep Brain Stimulation (DBS) done?
- Neurological assessment; movement tests on and off medication to measure the response to medicines
- Memory and mental health assessment, brain MRI
- A joint decision with neurosurgery; planning the target area with MRI and CT
- Placing the electrodes during surgery (in some centres testing while the patient is awake) and fitting the device under the chest skin in the same or a separate session
- Switching the device on a few weeks later and programming by a neurologist over months
