What is TMJ (Jaw Joint) Treatment?

The temporomandibular joint (TMJ) is the joint just in front of the ear where the lower jaw attaches to the skull. Jaw joint disorders cover different problems: pain and tension in the chewing muscles, clicking or locking caused by the disc inside the joint moving out of place, and arthritis of the joint. Stress, clenching and grinding, injury and some joint diseases can play a part. The basis of treatment is conservative care, which is enough for most patients: information and changing habits, a soft diet, jaw exercises, heat and cold, short courses of medication and splints made from impressions by the dentist. For those who do not improve with these steps, washing out the joint (arthrocentesis), injections into the joint or botulinum toxin into the muscles are considered; evidence for botulinum toxin in this use is limited. Open surgery is needed only in a small number of selected patients. A night guard that only protects the teeth from night-time grinding is not the same as a splint designed to treat the joint.

How is TMJ (Jaw Joint) Treatment done?

  1. A detailed history of when and how pain, clicking and locking happen
  2. Examination of jaw movements, muscles and how the teeth meet; panoramic X-ray, CT or MRI if needed
  3. Conservative treatment: changing habits, exercises, heat, short courses of medication and physiotherapy if needed
  4. Fitting and checking a custom stabilisation or repositioning splint
  5. For cases that do not improve: arthrocentesis, joint injections or, in selected patients, surgery
