What is shoulder Replacement Surgery?
Shoulder replacement is an operation in which the damaged surfaces of the round head of the upper arm bone (humerus) and the socket in the shoulder blade (glenoid) are replaced with artificial parts. An anatomical replacement copies the natural structure of the joint: a metal head and a plastic socket are placed; for it to work well, the tendons that rotate the shoulder (the rotator cuff) must be intact. In a reverse replacement, the ball and socket swap sides, so the deltoid muscle largely takes over lifting the arm. Reverse replacement is preferred especially for large irreparable rotator cuff tears, for complex fractures in older people and when previous operations have failed. Half replacement, in which only the head is replaced (hemiarthroplasty), is used in more limited situations today. The main aim of surgery is to reduce pain; the gain in range of movement depends on the type of replacement and the state of the muscles.
How is shoulder Replacement Surgery done?
- Examination, X-rays and usually CT to assess the bone and socket; MRI of the tendons if needed
- Anaesthesia assessment; general anaesthesia, usually with a nerve block in the neck
- Opening the joint through an incision at the front of the shoulder and removing the damaged surfaces
- Placing anatomical or reverse replacement parts, with or without cement
- An arm sling, pain control and planning physiotherapy
