What is scar Revision (Scar Treatment)?

Every deep wound in the skin heals with a scar; no method removes a scar completely. The aim is to make it thinner, flatter and more in keeping with the surrounding skin. Scars come in different types: raised hypertrophic scars that stay within the wound edges, keloids that grow beyond the wound edges, sunken (atrophic) scars after acne or chickenpox, contracted scars around joints that limit movement, and wide scars from poor stitching. Treatment is chosen according to the scar: silicone gel or sheets and pressure therapy, steroid injections into the scar, vascular or fractional lasers, microneedling, releasing the base of sunken scars (subcision) and filler, and, surgically, removing the scar and re-stitching it in a new direction (Z-plasty, W-plasty). Scars keep maturing for 12–18 months, so definitive surgical revision is usually planned once the scar has matured. If a keloid is treated with surgery alone, it can come back larger.

How is scar Revision (Scar Treatment) done?

  1. Assessing the scar's type, age and site and the person's tendency to keloids
  2. Discussing non-surgical treatments first or a combined treatment plan
  3. Sessions of injections, laser or microneedling at suitable intervals
  4. If surgery is needed, removing the scar under local anaesthesia and layered stitching, with a direction-changing flap technique if needed
  5. Scar care afterwards with silicone, pressure and sun protection
