What is bladder Botox Injections?

Overactive bladder shows itself as a sudden urge to pass urine that cannot be put off, passing urine often, getting up at night to pass urine and sometimes leaking before reaching the toilet. Botulinum toxin can be injected into the bladder muscle in people who have not improved enough with bladder training, fluid adjustment and medicines, or who cannot tolerate the side effects of medicines. During the procedure a thin camera (cystoscope) is passed through the urethra into the bladder, and small amounts of medicine are injected into many points in the bladder wall. The medicine temporarily reduces the nerve signals that trigger the muscle to contract, so urgency and leakage episodes decrease. The effect starts within a few days to two weeks and is not permanent; when it fades over months, the treatment can be repeated. It is also used for neurogenic bladder due to multiple sclerosis or spinal cord injury. Its main drawback is that in some patients the bladder does not empty completely and self-catheterisation may be needed for a while; this possibility should be accepted before the procedure.

How is bladder Botox Injections done?

  1. Bladder diary, urine test and culture; measuring urine left in the bladder
  2. Urodynamics and cystoscopy if needed to rule out other causes
  3. Teaching or information about intermittent self-catheterisation before the procedure
  4. Injections into the bladder wall through a cystoscope under local anaesthesia, sedation or general anaesthesia
  5. Short observation, checking you can pass urine and a check-up after a few weeks to measure urine left in the bladder
