What is neurological Rehabilitation?
Neurological rehabilitation is a planned treatment process to regain movements and functions lost through damage to the brain, spinal cord or nerves, or to adapt to permanent losses. After the brain is damaged, the healthy areas can form new connections to some extent through repetitive, task-focused work; this is called neuroplasticity. The programme is therefore based on intensive, repeated exercise built around the patient's own goals (walking, dressing, climbing stairs, holding a pen). The physical medicine and rehabilitation doctor plans the programme and manages medical issues such as muscle stiffness (spasticity), pain and swallowing problems; the physiotherapist works on movement and balance, the occupational therapist on daily living skills, and the speech and language therapist on speech and swallowing. Recovery is usually fastest in the first three to six months, but meaningful gains are also possible in later months and years. How much function can be regained depends on the site and size of the damage and cannot be predicted exactly in advance.
How is neurological Rehabilitation done?
- Examination by a physical medicine and rehabilitation doctor; recording muscle strength, spasticity, balance, sensation and cognition with scales
- Setting realistic, measurable goals with the patient and family
- A staged programme starting with positioning in bed, sitting balance and transfer training
- Walking, balance, arm and hand use and task-focused exercises; robotic or virtual reality-assisted work in suitable patients
- Home adaptations, choosing assistive devices and training carers
