What is pelvic Organ Prolapse Surgery?

Pelvic organ prolapse is when the muscles and connective tissue that hold the womb, bladder and end of the bowel in place loosen, and these organs drop into the vagina. Prolapse of the bladder is called a cystocele, and the rectum bulging into the back wall of the vagina a rectocele; the womb can drop on its own or together with these. A feeling of a bulge or lump in the vagina, heaviness pulling down, not emptying the bladder fully and straining to open the bowels are common complaints. For mild prolapse, pelvic floor exercises or a support ring placed in the vagina (pessary) may be enough. For advanced prolapse that causes symptoms, surgery is planned: options include repairs of the front or back wall through the vagina, removing the womb and suspending the top of the vagina from ligaments, suspension procedures that keep the womb, or fixing the top of the vagina to the front of the tailbone with a synthetic strip by keyhole surgery (sacrocolpopexy). The use of synthetic mesh through the vagina has been restricted in many countries because of reported complications; this should be discussed openly before surgery. If there is urinary leakage together with prolapse, it can be assessed with tests such as urodynamics and treated in the same session if needed.

How is pelvic Organ Prolapse Surgery done?

  1. Determining on examination which compartment has prolapsed and how far
  2. Urine test, ultrasound and urodynamics if needed for leakage and emptying problems
  3. Choosing the method according to pregnancy wishes, sexual life and general health
  4. Repair through the vagina, laparoscopically or robotically under spinal or general anaesthesia
  5. A urinary catheter and vaginal pack for a few days if needed; discharge once bladder emptying is checked
