What is pelvic Floor Physiotherapy?

The pelvic floor is made up of muscles and ligaments that close the bottom of the pelvis like a hammock and support the bladder, womb and end of the bowel. With pregnancy and vaginal birth, chronic constipation, heavy lifting, menopause and age, these muscles can weaken; urinary leakage when coughing, sneezing or running, a feeling of prolapse and difficulty controlling wind can appear. In some people, however, the problem is not weakness but muscles that are too tight; this shows as painful sex, pelvic pain and difficulty passing urine. So rather than "Kegels for everyone", how the muscle works is assessed first. Evidence shows that supervised pelvic floor muscle training is effective as first-line treatment, especially for stress urinary incontinence. The programme is run in line with the assessment of a gynaecologist or urologist.

How is pelvic Floor Physiotherapy done?

  1. A detailed history: births, bladder and bowel habits, a bladder diary and the effect of complaints on daily life
  2. With the patient's consent, assessing pelvic floor muscle strength, endurance and ability to relax
  3. Teaching correct contraction and relaxation; support with biofeedback or electrical stimulation if needed
  4. A staged exercise programme working with the breath and the abdominal and hip muscles
  5. Bladder training, fluid and toilet habits, and setting up the home programme
